Choosing the Right Strength of Sildenafil

When deciding on the appropriate strength of Sildenafil, you must consider factors like the severity of your erectile dysfunction (ED), how well your body tolerates the medication, and any underlying health conditions you may have. Sildenafil is available in three strengths: 25 mg, 50 mg, and 100 mg.

How it works

Sildenafil is a PDE-5 inhibitor. It works by blocking phosphodiesterase type-5 which helps to relax and widen the blood vessels to the penis. This allows an increase in blood flow to the penis, making it easier to maintain an erection when sexually aroused.
To note: Sildenafil only works when sexually aroused and will not help you get an erection without stimulation.

Is Sildenafil safe?

Sildenafil has been widely studied and is considered safe for most people when used as directed and has been proven to be safe to take for a long time. There do not seem to be any lasting harmful effects from taking it for many months and years. However like all medication it can come with risks and it can cause: headaches, flushing, upset stomach, nasal congestion, and dizziness. If you have previously been diagnosed with any heart problems, take a nitrate, have severe liver impairment, or have a history of stroke or heart attack it may not be suitable for you. If you are a healthy individual it is unlikely to cause you any problems outside of the minor side effects.

How to take Sildenafil

Sildenafil is usually taken approximately 1 hour before sexual activity. It usually has a starting dose of 50mg, which can then be adjusted according to response. It is recommended to try Sildenafil a few times before trying something else. If you find 50mg aren't producing enough of an effect, the dose can be increased to 100mg. On the other hand if you find you are experiencing side effects from the 50mg tablets, you can try the lower strength dose of 25mg which can still successfully treat ED.
Sildenafil usually takes between 30 minutes to 1 hour to work, but the onset of effect may be delayed if taken with food. Once taken it usually lasts 4 – 5 hours and you should not take more than one ED treatment in a 24 hour period.

Getting the best results

To achieve the best results from sildenafil, it’s important to use it correctly. Here are some tips to maximize the benefits of sildenafil:

  1. Take it in advance: Sildenafil should be taken about 30 minutes to an hour before sexual activity, allowing the medication to be absorbed and become effective.
  2. Window of effectiveness: The effects will last for about 4 to 6 hours when taken, so plan accordingly, keep in mind that it won’t cause an erection without sexual stimulation.
  3. Avoid big meals: While it can be taken with or without food, taking it after a high-fat meal (e.g., pizza, burgers) can slow down its absorption and delay its effects. For best results, take it on an empty stomach .
  4. Avoid alcohol: Alcohol does not interact with sildenafil directly but it can make it more difficuly to get an erection.

Tadalafil – can be taken up to 36 hours before sex

Cialis (Tadalafil) – can be taken up to 36 hours before sex
Spedra – can be taken up to around 4 hours before sex
Making lifestyle changes, such as excercising, stopping smoking can help overcome ED and help increase the chances of treatment being successful. Discussing any psychological factors such as stress or anxiety with a doctor or therapist can also help overcoming ED.
Another medication is also available called Alprostadil. This is a synthetic hormone that stimulates blood flow to the penis. It comes in the form of and injection directly into the penis or as a small pellet placed inside the ureathra. Other help with ED

As you get older your risk for ED increases. But having trouble maintaining an erection isn’t always due to age with many men experiencing ED at some point in their lives. The good news is ED will usually go away with treatment and is usually easily treated with minimal side effects.
Identifying the cause of ED can help improve treatment outcomes. Psychological factors such as stress and anxiety are the most common cause of ED. Easing stress and anxiety can help reduce ED as well as improve overall health.

Common causes include:

  1. Heart disease
  2. High blood pressure
  3. High cholesterol levels
  4. Obesity
  5. Excessive alcohol consumption
  6. Smoking
  7. Low testosterone

Treating the conditions listed above can help with your ED problem and may reduce the need for treatment.
Therapy can help with stress or anxiety induced ED and these include:
Counseling: A therapist will help to identify and address any major stress or anxiety, helping you manage them.
Cognitive behavioural therapy (CBT): A therapist will work with you to help you manage your problems by changing the way you think and behave.
Sex therapy: A sex therapist will listen to you describe your problems and assess whether the cause is likely to be psychological, physical or a combination of the two. They can also help with worries caused by a lack of knowledge surrounding sex and addressing any emotional issues.
Other ways of reducing stress and anxiety include alternative therapies such as:
Yoga
Meditation
Relaxation techniques
Research shows these treatments can help manage and reduce your stress and anxiety levels.
There is less evidence supporting these alternative therapies compared to more traditional therapies, however they may provide additional benefits when used alongside any ED treatments you are prescribed.

Sildenafil vs Viagra

Viagra is simply the brand name for Sildenafil, which was first made by Pfizer, since it is no longer under patent other drug companies can now manufacture Sildenafil tablets at a much lower price. Despite the name difference both tablets contain the exact same active ingredient, which is Sildenafil. Viagra is more expensive than Sildenafil as you are paying for the brand name rather than it providing a better or faster response.

Sildenafil side effects

As with any medication you take, there is a possibility of side effects occurring, most men take Sildenafil and experience no side effects with less than 2% of men reporting they experience a side effect. Any side effects you do experience is likely to be mild. The most common side effects of Sildenafil include headaches, flushing, nasal complaints (stuffy or runny nose), nausea, vomiting or diarrhoea. The more serious side effects include:
An erection that lasts more than 4 hours - this can lead to damage to your penis
Breathing difficulties, dizziness, swelling of face, lips, or tongue - you may be having an allergic reaction
Chest pains
Sudden vision loss in one or both eyes - this sign of a serious eye problem called non-arteritic anterior ischemic optic neuropathy
Sudden hearing decrease or hearing loss - some people may also have ringing in their ears (tinnitus) or dizziness

Who cannot take Sildenafil

Sildenafil is well tolerated and suitable for most men over the age of 18. However it may not be suitable if you:

  1. Have previously had an allergic reacion to Sildenafil
  2. Are taking nitrates for chest pain
  3. Suffer with a serious heart or liver condition
  4. Have recently had a stroke, heart attack or other heart problem
  5. Suffer from low blood pressure
  6. Have a rare inherited eye disease
  7. Suffer from sickle cell anaemia, leukaemia or multiple myeloma
  8. Have a penis defomity or a curved penis (Peyronie's disease)
  9. Have a stomach ulcer
  10. Have a bleeding disorder such as haemophilia

Drug Interactions with Sildenafil

The following drugs interact with Sildenafil, make sure you tell your doctor if you are taking:
Nirates eg. Isosorbide mononitrate
Poppers or other nitric oxide donors
Protease inhibitors eg. Ritonavir
Alpha-blockers eg. Tamsulosin, doxazosin.
Riociguat
You should also be aware that any herbal or alternative remedies you take may also interact with Sildenafil and cause side effects. If you are taking any herbal or alternative remedies make sure to mention them to your doctor, or on the consultation form so we can make sure they are safe to take together. Sildenafil combined with other erectile dysfunction treatments

Sildenafil should not be combined with other PDE5 inhibitors like tadalafil (Cialis), vardenafil (Levitra), or avanafil (Stendra). As these medication all work very similarly and are all PDE5 inhibitors, taking them all together can lead to an overdose, significantly increasing the risk of severe side effects, including a dangerous drop in blood pressure. This is why you should always leave at least 24 hours bewteen trying a different medication to treat erectile dysfunction.
Other treatments such as lifestyle changes, counselling and dietary changes can also be done alongside taking sildenafil and can improve treatment outcomes.

Sildenafil and graprefruit juice

It may sound strange but grapefruit juice can increase the levels of sildenafil in the bloodstream. This happens because grapefruit juice inhibits the action of an enzyme called CYP3A4 in the liver and intestines. This enzyme is responsible for breaking down sildenafil in your body. Therefore when grapefruit juice is consumed, it will inhibit and reduce the activity of CYP3A4, leading to higher levels of sildenafil in your bloodstream. This could potentially lead to an increased risk of side effects. To prevent these interactions, it's best to avoid grapefruit and grapefruit juice while taking sildenafil.

Manufacturers of Sildenafil

As Sildenafil is a generic medicine there are many different manufacturers we usually stock: TEVA, Accord, Dr Reddys and Milpharm. Although they are all different brands the manufacturers must all meet the same MHRA requirements and therefore they contain the exact same amount of active ingredient, this means that TEVA Sildenafil, Accord Sildenafil, Milpharm Sildenafil etc. should all work the same as eachother.
However some people do prefer specific brands and in some cases find they work better. We may not have each of these brands in stock when you order, so if you require a specific brand please check with us first via email. If for example you wanted TEVA Sildenafil tablets and it wasn't in stock we may be able to order it specifically for you but this will likely result in a one day delay to your order as we wait for them to come into stock.

Sildenafil vs Tadalafil (Cialis)

Tadalafil or Cialis is another form of medication that is used to treate ED. Like Sildenafil it starts to work in around 30 minutes, but lasts much longer than Sildenafil does with an effect seen for up to 36 hours. For this reason it is known as the 'weekend' pill. This does not mean you will have an erection that lasts 36 hours more that you will be able to get and maintain an erection when sexually aroused in a 36 hour period. This longer duration can be a drawback if you experience side effects as the drug will remain in your system for up to 36 hours compared to Sildenafil which will begin to wear off after 4 hours.
Sildenafil has been around longer than Tadalafil but Tadalafil has been around as Cialias since 2003 when it was first manufactuered. Both are safe and effective treatements for ED. The use of either tablet tends to come down to personal preference.

Drowsiness

This medicine can cause dizziness and can affect vision in some people. If you have not previously taken this medication, take a dose first to see how you react before you drive or use machinery.

    Sildenafil should not be taken by those with heart conditions as it can widen the blood vessels putting increased pressure on the heart. If you have a heart condition please discuss with your doctor, as suitability can depend on multiple factors and circumstances.

  • Will Sildenafil help with premature ejaculation?

    Sildenafil will only with ED, it will not directly help with premature ejaculation (PE). Dapoxetine which has been proven to help with PE and is manufactured as the brand Priligy, this can help you last up to three times longer in bed.

  • Is there a herbal Sildenafil?

    Alcohol intake should be avoided or kept to a moderate amount when taking Sildenafil, as it can increase the risk of side effects and also reduce the ability for you to maintain an erection.
